Abstract

Introducao: O tratamento das vitimas de ferimentos por arma de fogo (FAF) em face representa um desafio ate mesmo para os cirurgioes mais experientes da area. A forma, tamanho, composicao e velocidade do projetil e a distancia da arma ate o alvo ditam o grau de destruicao destas lesoes. Objetivo: Relatar abordagem cirurgica de um paciente jovem do sexo masculino vitima de FAF em face, evoluindo com fratura cominuta do corpo mandibular esquerdo. Relato de caso: Paciente do sexo masculino, 24 anos, caucasiano, vitima de FAF em face evoluindo com fratura cominuta do corpo mandibular esquerdo, sem comprometimento das vias aereas. Sob anestesia geral e intubacao nasotraqueal, realizado reducao e fixacao da fratura de mandibula. No pos-operatorio de 60 dias, paciente se apresentava sem queixas, com melhora da abertura bucal, oclusao satisfatoria e sem quaisquer sinais de complicacoes. Discussao: Existe descrito na literatura diversas formas de tratamento para essa condicao. No caso descrito o procedimento de desbridamento foi realizado, tornando o prognostico mais favoravel ao se adotar medidas que evitem a instalacao de um quadro infeccioso. Conclusao: O uso de placas de reconstrucao do tipo load bering sao as melhores opcoes de placas para o tratamento de fraturas altamente cominuidas, pois dispoem de um aparato que resulta em grande estabilidade dos cotos osseos. Por fim, o acompanhamento pos operatorio rigido e indispensavel para um bom prognostico de casos que possuem alta taxa de complicacoes.,
