A HISTOPATOLOGIA DAS LESÕES CUTÂNEAS ESPECÍFICAS DO LÚPUS ERITEMATOSO

Abstract

Introdução: O lúpus eritematoso (LE) é uma doença inflamatória autoimune do sistema conjuntivo, cuja sintomatologia principal são as lesões cutâneas, caracterizadas por infiltração perivascular de monócitos com posterior envolvimento da epiderme e anexos. Sendo elas classificadas de acordo com sua histopatologia como específicas ou não-específicas, logo, compreender a histopatologia do LE cutâneo específico é fundamental para um melhor prognóstico desta doença. Objetivo: Discorrer sobre as características histopatológicas das lesões cutâneas específicas do LE, suas diferentes classificações e abordar as principais manifestações da doença. Material e métodos: A metodologia consistiu em uma revisão literária de artigos publicados nas bases de dados “PubMed” e “Scielo”, no período entre 2012 e 2022, tanto em língua inglesa quanto portuguesa. Resultados: Com base nos artigos apurados, foram expostos como as lesões cutâneas do LE são categorizadas em: aguda (LECA); subaguda (LECS); crônica (LECC). Apesar de convergir quanto à dermatite de interface com degeneração vacuolar da camada basal, elas divergem quanto às suas histopatologias específicas e manifestações clínicas. Entre as classificações, o LECA é a forma mais comum, habitualmente manifestada por uma lesão malar em forma de borboleta, sendo a histologia da lesão descrita por degradação da camada basal, infiltrado mononuclear na junção dermoepidérmica e hiperqueratose. Já a LECA possui como característica específica a necrose subepidérmica, além de uma menor infiltração inflamatória e nível de hiperqueratose reduzido quando comparada com a LECA. A LECA, cujo tipo mais frequente é a LE discóide, se caracteriza por atrofia pilossebácea, deposição de mucina entre fibras colágenas, que acarreta numa fibrose da derme, apresenta melanófagos com melanina na derme superficial, logo sua coloração diferencia-se das outras lesões. Conclusão: Ante a revisão exposta, torna-se evidente como o discernimento histopatológico das diferentes lesões cutâneas da LE é imprescindível ao correto diagnóstico desta doença. Tendo em vista seu amplo espectro sintomatológico, reconhecer suas variadas manifestações dermatológicas, ampara tanto nas avaliações iniciais quanto no tratamento terapêutico do paciente crônico.
