HISTOPATOLOGIA DO DESCOLAMENTO DE RETINA

Abstract

Introdução: A retina é um tecido especializado, de organização laminar, ou seja, é formada por várias camadas de células. Ela se divide em duas camadas básicas: A retina neural que é a camada interna com fotorreceptores (cones e bastonetes), e o epitélio pigmentado da retina, camada externa de células cúbicas que repousa sobre a corióide. A condição denominada descolamento de retina ocorre quando o espaço entre essas duas camadas se expande causando a separação da retina neural do epitélio pigmentar, que continua aderido na corióide. Objetivo: Conceituar, com base em sua histopatologia, os diferentes tipos de descolamento de retina e seus fatores de risco associados. Material e métodos: Trata-se de uma revisão bibliográfica realizada através das bases de dados Scielo e Pubmed, com estratégia de busca “Retinal Detachment AND Vitreous Detachment” e seus respectivos em português, publicados entre o ano de 2007 até 2022. Foram encontrados 10 artigos, dos quais 4 foram selecionados para revisão, levando em conta as palavras-chave relacionadas a busca. Adicionaram-se, também, informações do livro “Histologia - Texto e Atlas - 6ª Ed. 2012” ao estudo. Resultados: O colapso do humor vítreo é um processo que acontece normalmente com a idade e causa a formação de um espaço de separação entre a membrana hialóide posterior e a retina, denominada descolamento do hialóide posterior, se a mesma não se separar livremente da membrana limitadora interna da retina, o vítreo exercerá tração sobre ela e a mesma por sua vez será rompida. Nisso o vítreo se infiltra através do rasgo e a retina neural é separada do epitélio pigmentar, caracterizando um descolamento de retina regmatógeno, ou seja, com ruptura. O descolamento de retina também pode ser não regmatógeno, sem ruptura da retina, geralmente é secundário à tumores e processos inflamatórios. Conclusão: A literatura indica que os fatores de risco incluem: Retinopatia diabética proliferativa, Miopia extrema, e paciente com histórico de cirurgia ocular ou lesões oculares graves. Além disso, o envelhecimento é apontado como principal fator de risco para o descolamento de retina, devido à mudança na composição química do humor vítreo nessa fase.
