USO DE HIPOGLICEMIANTES ORAIS EM PACIENTES COM DIABETES MELLITUS TIPO 2 E SUA RELAÇÃO COM A DOENÇA PERIODONTAL

Abstract

Introdução: O Diabetes Mellitus tipo 2 (DM2) é uma doença metabólica crônica relacionada ao aumento da glicemia devido a resistência à insulina. Quando descontrolada, pode causar diversas complicações sistêmicas, inclusive na cavidade oral. A doença periodontal (DP) é uma patologia que consiste na infecção crônica do periodonto relacionada com o biofilme dental. Os hipoglicemiantes orais são os fármacos de escolha do tratamento da DM2. As drogas utilizadas podem ser do grupo das sulfonureias, biguanidas, glitazonas e meglitinidas. Objetivo: Determinar o impacto do uso de hipoglicemiantes orais na prevenção da manifestação ou agravo de doenças periodontais em pacientes com Diabetes Mellitus. Material e métodos: Revisão bibliográfica integrativa através de pesquisa em banco de dados científicos como LILACS, Google acadêmico e SCIELO buscando artigos relacionados ao tema publicados entre 2017 e 2022. Resultados: A relação entre o diabetes e a doença periodontal já vem sendo descrita nos últimos 50 anos. Sabe-se que o diabetes descontrolado pode desencadear a doença periodontal ou agravá-la. Já a doença periodontal pode prejudicar o tratamento e controle da diabetes. Os produtos finais da glicação na hiperglicemia ativam macrófagos e levam a produção de citocinas inflamatórias que aceleram a destruição do periodonto. Estudos mostram maior prevalência de DP em pacientes diabéticos que não fazem uso de hipoglicemiantes orais. Estes medicamentos reduzem a glicemia em pacientes com DM2 por mecanismos de ação diversos como aumento da secreção de insulina, redução da absorção da absorção intestinal da glicose e estimulação dos receptores de insulina nos tecidos. Destaca-se a metformina com estudos que sugerem sua atuação na redução da perda óssea alveolar. Conclusão: Pacientes com DM2 controlada com o uso de hiperglicemiantes orais tem menor risco de desenvolver ou agravar DP. Por outro lado, o devido tratamento e controle da DP ajuda no controle da DM2. Assim, existe uma associação epidemiológica entre ambas doenças evidenciando a importância da elaboração de um plano de tratamento multidisciplinar envolvendo o cirurgião-dentista, médicos e outros profissionais de saúde que acompanham este paciente.
