Future prospect for ambulatory electrocardiography.

Abstract

ホルター心電図の連続記録時間を, 現行の24時間から大幅に延長 (数日, 数週, 数月) することの臨床的意義と技術的可能性につき, まず論じた.すなわち診断・治療の不十分あるいは過剰を防ぐ意味で, 記録時間の長期化は望ましいこと, その実現には装置の縮小・軽量化が前提になることを指摘した.かかる技法には心電図信号処理機構の組み込みによる実時間処理方式の実用化が必要になり, それができれば異常事態発生に際しての警報発生, さらには, 緊急対応の機能 (ペースメーカ, 除細動器, 注射器などとの組み合わせによる) も発揮し得ることを論じた.このような心臓監視・治療の自動化システムは, 携帯式あるいは内蔵式の装置として利用され, ホルター心電図の将来方向を示していると結論した.なお, 上述の夢の装置の内蔵化の場合のテレメータ機能, 内蔵式, あるいは携帯式の場合の超長時間利用 (慢性疾患の治療・制御) , さらには携帯式での外来患者診断への応用の可能性についても言及した.
