Abstract

Introducción: La neoplasia mieloproliferativa filadelfia negativa de peor pronóstico es la mielofibrosis primaria, está se caracteriza por una expansión monoclonal de líneas celulares, que, a diferencia de la leucemia mieloide crónica, que es filadelfia positiva, estas poseen más mutaciones como el gen Jak2. La estadística internacional arroja incidencia de 0.5 -1.5 casos por cada 100.000 individuos por año. En Venezuela no contamos con estadísticas de la incidencia de esta patología. Caso clínico: Se presenta paciente femenino de 50 años en regulares condiciones, con dolor abdominal generalizado, diaforesis, intolerancia de vía oral, e ingresa con diagnóstico de abdomen agudo quirúrgico, es llevada a mesa operatoria donde se realiza esplenectomía abierta. Conclusión: Numerosos estudios han demostrado que la realización de esplenectomía en pacientes con criterios para el mismo, brinda mejoras en la anemia y evita posibles complicaciones que representa este diagnóstico. Aunque es infrecuente la incidencia de mielofibrosis primaria, es necesaria la realización de publicaciones a mayor escala para determinar el abordaje adecuado de esta neoplasia en vista de su alta morbi-mortalidad y escasas opciones terapéuticas. En este caso la realización de la esplenectomía, evidenció mejoría de los síntomas, obteniéndose, cese de dolor abdominal secundario a visceromegalia y mejora de la anemia
