Papiloma invertido nasossinusal simulando estesioneuroblastoma

Abstract

Introdução: Os tumores nasais, paranasais e da base do crânio compõem lesões raras e geralmente agressivas. A clínica comumente é inespecífica e insidiosa, tendendo a levar estes pacientes a um mau prognóstico. Neste contexto, uma vez que no seu diagnóstico e no planejamento cirúrgico tornam-se imperativos a realização da biópsia e dos exames de imagem, descrevemos um relato de caso na qual um papiloma invertido (PI) simulava nas suas imagens radiológicas um estesioneuroblastoma (neuroblastoma olfatório) (ENB), com o objetivo de contribuir no diagnóstico diferencial de ambas as doenças. Relato de caso: Uma mulher de 69 anos queixava-se de obstrução nasal, cefaleia frontal e proptose à esquerda, de maneira que a rinoscopia evidenciou uma lesão expansiva na fossa nasal ipsilateral. Aos exames de tomografia computadorizada (TC) e de ressonância magnética (RM), foi demonstrada uma lesão solidocística em haltere com envolvimento das fossas craniana anterior e nasal esquerda, centrada na placa cribriforme e associada a cistos periféricos intracranianos, achados clássicos de um ENB. A paciente foi submetida a frontoetmoidectomia esquerda com exérese do tumor nasal, mas a biópsia, no entanto, revelou tratar-se de um PI. Conclusão: Apresentamos um relato de caso de um PI na qual sua imagem simulava um ENB, tratando-se de uma manifestação atípica de uma doença rara. Espera-se, diante do exposto, que este trabalho colabore no auxílio ao diagnóstico diferencial de ambas as doenças.
