Abstract

La detección de autoanticuerpos en las enfermedades reumatológicas autoinmunes sistémicas denominadas colagenopatías es de gran valor para apoyar el diagnóstico clínico, y en ocasiones permite detectar y clasificar subtipos. Los actuales trabajos buscan la posibilidad de utilizar biomarcadores de las distintas enfermedades como predictores de la evolución clínica. Por ejemplo, intentan integrar marcadores clásicos de activación, como el componente 3 del complemento (C3) con autoanticuerpos como los anti-dsDNA o los anti-Sm en el lupus eritematoso sistémico. La técnica de inmunofluorescencia indirecta sobre las células HEp-2 es la más utilizada como tamizaje. Detecta la presencia de autoanticuerpos, brinda una idea de su cantidad al titularla y además el patrón en ocasiones permite inferir cuál es el antígeno blanco que luego debe corroborarse con otras técnicas. En esta revisión describimos la nueva taxonomía propuesta por un conjunto de expertos en 2014 y actualizada en septiembre 2021 enfatizando el correlato clínico y la utilidad de los principales anticuerpos anticelulares. El objetivo es comunicar los conceptos actuales con esquemas y tablas que puedan resultar útiles para los profesionales en su práctica.
