Caracterización clínica de pacientes con hemorragia gastrointestinal superior

Abstract

OBJETIVO: describir las características clínicas de pacientes con hemorragia gastrointestinal superior en los servicios de medicina interna del Hospital de Chiquimula durante los meses de enero 2020 a diciembre 2024 y ortopedia del Hospital de Chiquimula en los meses de mayo a junio del 2025. MÉTODO: se realizó un estudio descriptivo retrospectivo de 93 expedientes clínicos. RESULTADOS: el 51.61% de los pacientes fueron mujeres y el 48.39% hombres. Los grupos etarios más frecuentes fueron de 51–55 años con 12.90% y 71–75 años con 12.90%. Las manifestaciones clínicas predominantes fueron hematemesis en 89.25%, palidez en 81.72%, melena en 60.21%, taquicardia en 60.21%, dolor abdominal en 58.06%, hipotensión en 30.10% y hematoquecia en 9.68%. Los factores de riesgo más frecuentes fueron uso de AINES con 52.69%, alcoholismo en el 51.61%, tabaquismo en el 41.93% y cirrosis hepática en el 29.02%. La hemorragia fue variceal en el 49.46% y no variceal en el 50.54%, donde las várices esofágicas se presentaron más, úlcera gástrica en el 23.65% y gastropatía erosiva en el 15.05%. El tratamiento fue médico en el 58.06% y endoscópico en el 41.94%. CONCLUSIÓN: la hemorragia gastrointestinal superior en los pacientes del Hospital de Chiquimula se caracterizó factores de riesgo como el uso de AINES y el alcoholismo. Las principales causas fueron úlceras y várices esofágicas, tratadas mediante manejo médico y procedimientos combinados.
