Grado de severidad en pacientes sometidos a colecistectomía laparoscópica

Abstract

OBJETIVO: describir el grado de severidad en pacientes sometidos a colecistectomía laparoscópica según la escala de Parkland en el Departamento de Cirugía del Hospital General de Enfermedades del Instituto Guatemalteco de Seguridad Social en la ciudad de Guatemala en los meses de mayo y junio del año 2025. MÉTODO: se llevó a cabo un estudio descriptivo de tipo transversal en un universo de 53 pacientes intervenidos mediante colecistectomía laparoscópica. RESULTADOS: el 45% de los pacientes presentó grado 2 en la escala de Parkland, seguido de 21% con grado 1, 19% con grado 4, 11% con grado 3 y solo el 4% con grado 5. El 58% de los pacientes correspondió al sexo masculino, el grupo etario predominante fue el de adultos jóvenes con 58%. En cuanto a los cólicos biliares previos, el 34% reportó tres episodios. El 88% no presentó complicaciones postoperatorias. El 66% de las complicaciones ocurrió en pacientes con Parkland grado 2, el 17% en grado 3 y el 17% en grado 4. En enfermedades concomitantes, la hipertensión arterial fue la más frecuente con 21%, seguida de diabetes mellitus con 8% y otras condiciones como hipotiroidismo, obesidad, pancreatitis, lesión renal aguda, cáncer de colon, gastritis e insuficiencia cardíaca, cada una con un 2%. CONCLUSIÓN: la escala de Parkland resultó útil para estimar la complejidad quirúrgica, lo que destaca su valor como herramienta de estratificación intraoperatoria en la colecistectomía laparoscópica.
