To close or not to close?

Abstract

Ein 48-jahriger Mann konsultierte in seinen Ferien in der Schweiz wegen Fieber bis 39° und Schuttelfrost ohne weitere Begleitsymptome den Notfallarzt. Klinisch konnte kein Infektfokus eruiert werden, das C-reaktive Protein (CRP) war jedoch mit 154 mg/dl deutlich erhoht, und es fand sich im Blutbild eine Linksverschiebung bei normaler Leukozytenzahl. Aus der personlichen Anamnese war zu erfahren, dass der Patient Monate zuvor wegen einmaliger atypischer Thoraxschmerzen kardiologisch abgeklart worden war. In der transosophagealen Echokardiographie wurde damals ein Vorhofseptumaneurysma mit offenem Foramen ovale (PFO) diagnostiziert. Bei dem passionierten Taucher wurde daraufhin gut drei Monate vor dem jetzigen Ereignis ein perkutaner Verschluss des PFO mit einem Occlutech Figulla Occluder N® (Abb. 1 x) durchgefuhrt. Angesichts dieser Vorgeschichte wurde der Patient bei Verdacht auf Endokarditis zur weiteren stationaren Abklarung und Behandlung zugewiesen. Bei Spitaleintritt zeigte sich der Patient in einem guten Allgemeinzustand, er war mit 37,8 °C subfebril, die klinische Untersuchung war ansonsten unauffallig. Die weiteren Laboruntersuchungen ergaben ein CRP von 237 mg/dl, eine Leukozytose von 10,7x109 mit Linksverschiebung von 37% und toxischen Zeichen sowie eine Thrombozytopenie von 115x109. Weiter ergab die Urinuntersuchung Hinweise fur eine Glomerulonephritis (Proteinurie, glomerulare Erythrozyten), welche als Begleitphanomen des aktuellen Leidens interpretiert wurde. In 10/10 Blutkulturen liess sich Staphylococcus lugdunensis nachweisen. Die transosophageale Echokardiographie zeigte auf dem korrekt positionierten PFO-Verschlussschirm eine unregelmassige, wenig echodichte Auflagerung von 2,8 mm, welche differentialdiagnostisch als Vegetation oder Thrombus intepretiert wurde (Abb. 2x). Bei hochgradigem Verdacht auf eine Device-Endokarditis erfolgte die stationare Aufnahme zur intravenosen resistenzgerechten Antibiotikatherapie mit initial Garamycin, Rifampicin und Flucloxacillin und im Verlauf Cefazolin fur insgesamt sechs Wochen. In der Folge zeigte sich ein komplikationsloser Verlauf mit vollstandiger Regredienz der Infektparameter, der Patient blieb afebril und beschwerdefrei. Im Verlauf entnommene Blutkulturen waren negativ. Der Patient wurde klinisch sowie mittels Blutkulturen eine Woche nach Beendigung der Therapie nachkontrolliert.
