Enhanced fibrinolysis in patients undergoing regular hemodialysis treatment

Abstract

安定期透析患者における線溶活性を新しい線溶の指標であるα2プラスミン・インヒビター (α2PI), α2PI-プラスミン複合体 (α2PIC), FDP-D dimerを用いて検討した. 安定期透析患者27例 (男9, 女18, 平均年齢42歳, 平均透析歴85か月, 原疾患は全例慢性糸球体腎炎) を対象とし, 血液透析 (HD) 前にα2PICを測定, さらに15例についてHD前, 開始後30, 60, 120分, 終了時にフィブリノゲン (FBG), プラスミノゲン活性, AT III活性 (AT III), α2PI, α2PIC, D-dimerを測定した. 結果は, (1) プラスミノゲン活性, AT III, FBGはHD中に有意の変化を観察しなかった. (2) α2PI活性は前91.2±2.6 (m±SE) に対し終了時85.7±3.7%と有意に低下した (P<0.05). (3) α2PICの前値は1.15±0.16μg/mlで, 正常より有意の高値を示した (P<0.01). (4) α2PICのHD中の変化は前値0.93±0.12, 120分1.29±0.20, 終了時1.72±0.23μg/mlと, HD経過とともに上昇, 120分, 終了時は前値に比べ, 有意の上昇であった (それぞれP<0.005, P<0.001). (5) D-dimerは前61.9±4.74, 60分127.6±30.7, 終了時82.5±10.4ng/mlと60分をピークとする有意の上昇を示した (P<0.05). α2PI抗原量は60分に有意の低下を示した (P<0.05). 以上により, 安定期透析患者では線溶の亢進があり, HD中さらに線溶が亢進することが示唆された. またD-dimerも上昇することから, HD中の線溶亢進に二次線溶も関与することが推測された.
