Four chronic hemodialysis patients whose intact-PTH level markedly decreased after administration of saccharated iron.

Abstract

鉄欠乏状態にあり, 貧血治療の目的で静注鉄剤を投与したところ, 血中intact-PTHレベルの顕著な低下を認めた4例を経験したので報告する. いずれも透析歴は4-9年と長期の維持透析を行っており, intact-PTHは145-1,050pg/mlと高く, うち2例では血清ALP活性, 血中Piともに高値であり, 二次性副甲状腺機能亢進症 (2°HPT) の状態と考えられた. 鉄剤投与によりintact-PTHは, 43-90%の低下を認め, 全例で投与中止後再上昇した. 進行した2°HPTの決定的内科的治療法のない現在, 鉄剤投与が今後一つの補助的方法として検討されるべきであろうと考えた.
