De la croissance moyenne à la croissance optimale

Abstract

Partant d’une recherche sociologique conduite sur la base de corpus experts, scientifiques et politiques entre 1990 et 2005, cet article explore les contextes et modalités de fabrication des nouvelles courbes de référence pour la croissance des nourrissons et jeunes enfants de l’OMS. Il souligne à quel point ce projet, initialement pensé dans les arènes expertes, induit un glissement paradigmati­que fondamental : la mesure de la croissance moyenne d’enfants de milieux sociaux diversifiés laisse place à la recherche d’une croissance physiologique optimale, engagée chez les enfants de milieux sociaux aisés, proches des recommandations nutritionnelles de l’OMS. Il invite, plus largement, à interroger les recompositions de la biopolitique des populations en actes dans le champ de la santé publique internationale autour de quatre dimensions : une globalisation assise sur la quête d’invariants biologiques universalisables, un durcissement de la frontière entre déterminants biologiques et sociaux dans la vision de la croissance enfantine, une subordination du social au biologique (le premier est utilisé pour révéler un potentiel physiologique de croissance optimal) et un souci d’optimisation somatique dont les corps maternels allaitants constituent un levier d’action central.
