Abstract

ワルファリンは用量に著しい個人差があり,様々な薬物,食品との相互作用以外にも個々で様々な程度の耐性が存在しているといっても過言ではない.逆にアジア人種ではワルファリンが効きやすく,出血合併症を起こしやすいという意見もある.通常ワルファリン耐性とは1日15 mg以上必要な場合と定義され,その原因はコンプライアンス不良やビタミンK過剰摂取などのよく知られた要因と,先天的および外因による肝代謝活性の差(pharmacokinetic resistance),感受性の差(pharmacodynamic resistance)に大別される.そしてワルファリンの効果の個人差および人種差の原因が近年の遺伝子解析により判明しつつある.ワルファリンは消化管から速やかに吸収され,ほぼ100%肝臓でcytochrome P450(CYP)複合体により代謝される.薬剤としては2つの光学異性体が等量含まれており,このうちS体は抗凝固活性の約7割を担い,主に酵素CYP2C9により短時間で代謝される.R体は代謝酵素が異なるため,肝代謝の差は主としてCYP2C9活性の差に依存していると考えられる.他方ワルファリンは肝臓内ビタミンKサイクルの阻害により抗凝固作用を発揮するが,その作用はサイクルの主要酵素であるvitamin K1 2,3-epoxide reductase(VKOR)の活性に大きく影響されるために,この酵素活性の差により感受性の大部分が規定されることになる.このような特徴から,ワルファリンの場合は他の抗血栓薬と異なり,抵抗性ではなく過敏性を呈することがほとんどである.また抗凝固療法のモニタリングの根幹を成すPT-INR算出にあたって,使用するプロトロンビン時間試薬による差異も無視できないことが明らかとなった.
