A CASE OF SPONTANEOUS RUPTURE OF THE EXTERNAL ILIAC VEIN
特発性外腸骨静脈破裂に対し緊急手術を施行し,救命しえた1例を報告する.症例は79歳,女性.左下腹部痛を主訴に当院を受診した.腹部CTにて後腹膜血腫を認めた.患者は一旦心肺停止に陥ったが,速やかに蘇生され,緊急手術を施行された.後腹膜血腫の原因は左外腸骨静脈の3cmの縦裂開であり,連続縫合閉鎖にて止血しえた.腹腔内を検索した限り,腸骨静脈破裂の原因病変を認めず,特発性外腸骨静脈破裂と考えられた.術後第62病日に転院した.特発性腸骨静脈破裂は,検索しえた限りでは国内外において17例が報告されているに過ぎない稀な疾患であるが,時に致命的な経過をとることがあり,後腹膜出血の鑑別診断として念頭におくべき疾患であると考えられた.
