Alveolar pressure during pressure support ventilation.

Abstract

圧支持換気(pressure support ventilation, PSV)の設定圧が肺内でどのように伝播するかを調べるため,PSV中の肺胞圧を測定した。雑種成犬6頭をペントバルビタールで麻酔し,フジシステムズ製内径8.5mmのユニベントチューブを気管挿管した。犬を側臥位とし肋間筋を分け,「閉胸式カプセル法」によりカプセルを肺に接着し,その圧を肺胞圧とした。気管内圧はユニベントチューブのオクルージョンカテーテルより導出した。回路内圧,気管内圧および肺胞圧は圧トランスデューサで測定し,同時にポリグラフで記録した。PSVは呼気終末陽圧(PEEP)5cmH2O,立上がり時間0.5secで,設定圧5,10cmH2Oの2通りを検討した。回路内圧波形は設定圧に達するとプラトーとなったが,気管,肺胞圧波形ではプラトー部分はなかった。気管と肺胞の圧差は設定圧によらず極めて小さかった。正常な犬で肺内の気道抵抗は,PSV時の圧の伝播にほとんど影響を及ぼさないと考えられた。
