Farmacomicrobiomica: el futuro de la medicina personalizada

Abstract

La farmacomicrobiómica surge tras demostrarse la capacidad de la microbiota de modificar la farmacocinética y la farmacodinamia y ante la imposibilidad de explicar la falta de eficacia de algunos medicamentos por mecanismos farmacogenéticos y farmacogenómicos. El objetivo de este trabajo es investigar la importancia de la microbiota sobre la variabilidad interindividual de la respuesta farmacológica a nivel farmacocinético, farmacodinámico y bidireccional, así como las modificaciones microbianas mediadas por fármacos y las intervenciones sobre medicina precisión o personalizada. Para ello se realiza una búsqueda general bibliográfica en PubMed, seguida de búsquedas específicas por grupos de fármacos y otras para profundizar sobre la farmacocinética y el papel de la microbiota en la medicina. Los resultados muestran posibles aplicaciones en la medicina personalizada intentando disminuir la toxicidad de fármacos como el irinotecán o 5-FU o aumentar la eficacia de gemcitabina y levodopa por mecanismos farmacocinéticos y de ciclofosfamida, cisplatino, oxaliplatino y anticuerpos anti-CTLA-4 por mecanismos farmacodinámicos; o bien determinar la respuesta frente a metotrexato, anticuerpos anti PD-1 y anti-CTLA-4, liraglutida, captopril y metformina, según la microbiota encontrada. Como conclusión, debería ahondarse en la búsqueda de intervenciones sobre medicina personalizada para mejorar las terapias. Con relación a investigaciones futuras, debería conocerse la flora basal y utilizarse grupos de estudio comparables para descartar sesgos y conseguir resultados más precisos.
