Ar laparoskopija yra tinkamas būdas šalinti dideles gimdas?

Abstract

Tyrimo tikslas. Apžvelgti laparoskopinių supracervikalinių histerektomijų (LSH) rezultatus ir įvertinti gimdos svorio įtaką LSH sėkmingumui. Metodai. Retrospektyviai išanalizuoti 187 pacienčių, kurioms buvo atlikta LSH 2016–2018 m. Vilniaus miesto klinikinėje ligoninėje, duomenys. Atvejai suskirstyti į dvi grupes pagal gimdos svorį (I grupė – ≥500 g (n=59), II grupė – <500 g (n=128)). Atlikta statistinė analizė. Skirtumas statistiškai reikšmingu laikytas, kai p<0,05. Rezultatai. Ilgesnė vidutinė operacijos trukmė (p<0,001) bei didesnis vidutinis intraoperacinis kraujo netekimas (p>0,05) nustatytas I grupėje, tačiau nebuvo statistiškai reikšmingos koreliacijos tarp didesnio gimdos svorio ir didesnės komplikacijų rizikos. Hospitalizacijos trukmė ir intraoperacinių komplikacijų skaičius reikšmingai nesiskyrė tarp dviejų tiriamųjų grupių. Išvados. Laparoskopinė supracervikalinė histerektomija – tai minimaliai invazyvus, saugus chirurginis gydymo būdas, tinkamas didelėms gimdoms šalinti.
