
Apresentação: Os cuidados paliativos são essenciais na abordagem de doenças crônicas avançadas, transcendendo o tratamento curativo para focar na qualidade de vida. Esta abordagem reconhece a família não apenas como um suporte, mas também como uma unidade de cuidado que vivencia o sofrimento. Objetivo: Analisar, por meio de uma revisão integrativa da literatura, o impacto dos cuidados paliativos na dinâmica familiar e o papel dos familiares como cuidadores de pacientes com doenças crônicas em estágio avançado. Material e Método: Trata-se de uma revisão integrativa da literatura, com buscas realizadas nas bases de dados SciELO, LILACS, PubMed e BVS. A amostra final foi composta por 8 artigos científicos que atenderam aos critérios de inclusão e abrangiam estudos sobre demência, insuficiência cardíaca e DPOC. Resultados: A análise dos estudos revela que as intervenções paliativas demonstram um impacto mais proeminente e significativo no bem-estar dos cuidadores familiares, com reduções notáveis da sobrecarga psicológica e de sintomas de ansiedade e depressão. Embora os efeitos nos desfechos primários dos pacientes sejam mais modestos, observam-se benefícios pragmáticos relevantes, como a redução de hospitalizações e de eventos adversos, que aliviam indiretamente o estresse familiar. A sobrecarga do cuidador foi identificada como um fenômeno multidimensional, influenciado por fatores pessoais e sistêmicos, bem como pela especificidade da doença. Conclusão: A família consolida-se como a unidade central do cuidado paliativo, e a eficácia dessa abordagem reside em sua capacidade de amparar o núcleo familiar em sua totalidade. A prática clínica deve direcionar-se à implementação de suportes customizados que atendam às necessidades específicas de cada contexto familiar e de cada patologia. Background: Palliative care is essential in the approach to advanced chronic diseases, transcending curative treatment to focus on quality of life. This approach recognizes the family not just as a support system, but as a unit of care that also experiences suffering. Objective: To analyze, through an integrative literature review, the impact of palliative care on family dynamics and the role of family members as caregivers for patients with advanced chronic diseases. Materials and Methods: This is an integrative literature review, with searches conducted in the SciELO, LILACS, PubMed, and VHL databases. The final sample comprised 8 scientific articles that met the inclusion criteria and covered studies on dementia, heart failure, and COPD. Results: The analysis of the studies reveals that palliative interventions demonstrate a more prominent and significant impact on the well-being of family caregivers, with notable reductions in psychological burden and symptoms of anxiety and depression. Although the effects on patients' primary outcomes are more modest, relevant pragmatic benefits are observed, including reduced hospitalizations and adverse events, which indirectly alleviate family stress. Caregiver burden was identified as a multidimensional phenomenon influenced by personal and systemic factors, as well as the specific disease. Conclusion: The family is the central unit of palliative care, and the effectiveness of this approach lies in its ability to support the family as a whole. Clinical practice should be directed towards implementing customized support that meets the specific needs of each family context and pathology.
