Factores asociados a enfermedades gastrointestinales de adultos en Ecuador

Abstract

El perfil de prevalencia de las enfermedades gastrointestinales en adultos cambia constantemente lo que afecta el enfoque diagnóstico. El objetivo de este estudio fue caracterizar el perfil de enfermedades digestivas en mayores de 18 años en un hospital de segundo nivel en una gran ciudad de Ecuador. Se realizó un estudio transversal, basado en registros médicos de pacientes atendidos por demanda espontánea o referidos a consultas de gastroenterología en un hospital de segundo nivel. Se analizaron los diagnósticos clínicos organizados según la clasificación CIE-10, antecedente de enfermedad acido péptica (EAP) o enfermedad por reflujo gastroesofágico (apG/ERGE), uso de medicamentos para estas patologías (AMed), consumo de tabaco o alcohol (T/A), síntomas actuales de esófago y estómago (SSee) y edad. Para identificar patrones se empleó el análisis de correspondencias múltiples y para las interacciones significativas un modelo saturado log-lineal. Fueron analizados 535 registros médicos, 57,8% fueron mujeres. Las patologías esofágicas y gástricas (CIE-10: K20-K31) fueron las más comunes y estuvieron en menores de 40 años, consumen T/A, tuvieron SSee y usan AMed. Los diagnósticos de enfermedades intestinales, colon y hernia (K35-K67) fueron en mayores de 40 años, no consumen T/A, no ApMed y no tuvieron apG/ERGE. Los diagnósticos de hígado, vesícula y páncreas (K70-K87) fueron personas que no tuvieron SSee, no consumen T/A y fue independiente de la edad. En conclusión, las consultas de patologías esofágicas y gástricas fueron las más frecuentes en gastroenterología y se asociaron al consumo de tabaco y alcohol, al antecedente de EAP o ERGE y consumo de medicamentos para estas dolencias.
