Síndrome de sobreposición entre hepatitis autoinmune y cirrosis biliar primaria: reporte de caso

Abstract

El síndrome de sobreposición se refiere a la coexistencia de dos patologías autoinmunes que comparten características clínicas, histológicas, serológicas y bioquímicas. En adultos se observa con mayor frecuencia la asociación de hepatitis autoinmune (HAI) y colangitis biliar primaria (CBP). Se presenta el caso de un varón de 54 años de edad, con un año de enfermedad, pérdida ponderal, ictericia y prurito. Su perfil hepático estuvo alterado, colestasis, anticuerpos antinucleares (ANA) positivo, anticuerpos antimitocondriales (AMA) negativo, serología viral negativa. La biopsia renal presentó hallazgos relacionados al síndrome de sobreposición HAI-CBP. Se concluye que el diagnóstico y tratamiento del síndrome de solapamiento de CBP-HAI es dificultoso y debe considerarse en el diagnóstico diferencial de las hepatopatías autoinmunes. Se recomienda la intervención multidisciplinaria para establecer el diagnóstico correcto y ofrecer estrategias terapéuticas favorables.
