Atualização na Fratura Cervical em Portadores de Espondilite Anquilosante

Abstract

A Espondilite Anquilosante (EA) é uma doença sistêmica, inflamatória soronegativa crônica, que acomete homens e mulheres na 3ª e 4ª décadas de vida, e em sua maioria positiva para o antígeno HLA-B27. Os principais acometimentos ocorrem na articulação sacroilíaca e na coluna vertebral, com massa inflamatória ou ossificação do disco intervertebral, afetando também estruturas ligamentares e faceta articular. A classificação da AOSpine avalia o mecanismo do trauma, o tipo de lesão, fratura e/ou deslocamentos da faceta, o status neurológico e modificadores indicando sinais de instabilidade e ajudando na conduta conservadora ou cirúrgica. Apresentamos um caso clínico de um paciente com EA em que foi necessária intervenção cirúrgica em que se optou pela artrodese via posterior C4 a T1 longa por causa do traço de fratura do corpo de C6, sem desvio e sem déficit neurológico. A reabilitação do paciente foi realizada com mobilização precoce na própria UTI, quando necessária, baseada na monitorização do estado clínico e laboratorial no primeiro dia pós-operatório e com fisioterapia para deambulação sem uso de órtese.
