Abstract

Введение. Хронический генерализованныйпародонтит (ХГП), как и пищевая аллергия, относится к коморбидной хронической патологии и часто сопровождается различными изменениями со стороны органов и тканей полости рта. В развитии и течении ХГП нарушения цитокинового баланса претендуют на ключевую роль. Направленность местных реакций при заболевании, организация клеточно-опосредованного иммунного ответа, эффективность противоинфекционной защиты зависит от баланса между секрецией про- и противовоспалительных цитокинов. Проблема наличия цитокинового дисбаланса, его особенностей, степени выраженности у пациентов с ХГП и сопутствующей пищевой аллергией практически не изучена, что и определило цель нашего исследования. Цель исследования. Определение содержания цитокинов ИЛ-1β, ИЛ-2, ИЛ-4 и ИЛ-6 в ротовой жидкости у пациентов с хроническим генерализованным пародонтитом на фоне сопутствующей пищевой аллергии и их роли в патогенезе данного заболевания. Результаты. Результаты исследования выявили статистически достоверно выраженную разбалансировку уровня всех исследуемых нами цитокинов в ротовой жидкости (РЖ) в обеих группах пациентов по сравнению с данными практически здоровых людей (p<0,05). У пациентов с ХГП и сочетанной пищевой аллергией имеется более выраженный дисбаланс в системе цитокиновой регуляции, чем у лиц с ХГП без сопутствующей аллергической патологии. Заключение. Проведение корригирующей терапии аллергической патологии позволит улучшить течение хронического пародонтита и добиться стабилизации воспалительного процесса в пародонте. Introduction. Chronic generalized periodontitis (CGP), like food allergy, belongs to a comorbid chronic pathology and is often accompanied by various changes in the organs and tissues of the oral cavity. In the development and course of CGP, disturbances in the “cytokine balance” claim to play a key role. The direction of local reactions in the disease, the organization of the cell-mediated immune response, the effectiveness of anti-infective protection depends on the balance between the secretion of pro- and anti-inflammatory cytokines. The problem of the presence of cytokine imbalance, its features, severity in patients with CGP and concomitant food allergies is practically not studied, which determined the purpose of our study. Purpose of the study. To determine the content of cytokines IL-1β, IL-2, IL-4 and IL-6 in oral fluid in patients with chronic generalized periodontitis against the background of concomitant food allergies and their role in the pathogenesis of this disease. Results. The results of the study revealed a statistically significant imbalance in the level of all the cytokines studied by us in the oral fluid (OM) in both groups of patients compared with the data of practically healthy people (p<0.05). Patients with CGP and combined food allergy have a more pronounced imbalance in the cytokine regulation system than patients with CGP without concomitant allergic pathology. Conclusion. Carrying out corrective therapy of allergic pathology will improve the course of chronic periodontitis and achieve stabilization of the inflammatory process in the periodontium.
