Type I Tympanoplasty: How I Do It

Abstract

Введение. Восстановление элементов среднего уха остается одним из наиболее актуальных вопросов в отохирургии. Условие многих операций, которое приводит к конечной цели хирургии, – это восстановление целостности барабанной перепонки. Цель. Представить собственную технику тимпанопластики I типа при обширных дефектах барабанной перепонки. Материалы и методы. 850 пациентов прооперировано по предложенной методике с 2012 по 2023 г., период наблюдения составил до 10 лет. Тимпанопластику выполняли с помощью хрящевой пластины толщиной 0,2–0,3 мм, из которой формировали ряд мобильных по отношению другу к другу фрагментов по собственной авторской методике. Для формирования тимпанопластического лоскута применяли хрящевую пластину с насечками из аутологичного хряща ушной раковины, козелка или аллогенного хрящевого материала. В статье имеется указание на предложенный авторами новый раствор на основе производных гуанидина для консервации хряща, который успешно апробирован в эксперименте. Результаты. Получен высокий клинико-анатомический результат и функциональный результат в отдаленном периоде. Пластина приобретала устойчивость в области костного кольца, но сохраняла высокую мобильность за счет своих фрагментов. Она позволяла создавать воздушность неотимпанальной полости необходимого объема. Так же легко формировался передний меатотимпанальный угол. Предлагаемый нами вариант оперирования пациентов с обширной перфорацией барабанной перепонки по описанной методике относительно прост для выполнения и может быть рекомендован для использования, потому что анатомические и функциональные результаты являются высокими и стабильными. Introduction. Restoration of elements of the middle ear remains one of the most important question in otosurgery. The condition of many operations that leads to the ultimate goal of surgery is to restore the integrity of the eardrum. Purpose. To describe own technique of tympanoplasty type I for extensive tympanic membrane defects. Materials and methods. Tympanoplasty was performed using a cartilage plate 0.2–0.3 mm thick, from which a number of fragments mobile to each other were formed according to own author’s method. To form the tympanoplastic flap, a cartilaginous plate with incisions from autologous cartilage of the auricle, a tragus or allogeneic cartilaginous material was used. The article contains an indication of the new solution proposed by the authors based on guanidine derivatives for cartilage conservation, which was successfully tested in an experiment. Results. A high clinical and anatomical result and a functional result in the distant period were obtained. The plate acquired stability in the region of the bone ring, but maintained high mobility at the expense of its fragments. It made it possible to create the air-filled of the neotympanic cavity of the required volume. The anterior meatotympanic angle was also easily formed. Our proposed option of operating on patients with extensive perforation of the eardrum according to the described method is relatively simple to perform and can be recommended for use because the anatomical and functional results are high and stable.
