Abstract

В последние десятилетия акцент в менеджменте пациентов, страдающих ретинобластомой, сместился с частого использования дистанционной лучевой терапии к широкому применению цитостатических препаратов и локальных методов лечения. Данная стратегия реализуется с целью уменьшения возникновения вторых злокачественных опухолей у данной категории пациентов, особенно это актуально при наличии наследственной ретинобластомы. В распоряжении специалистов, занимающихся лечением ретинобластомы, есть такие методы лечения, как системная и интраартериальная химиотерапия, криодеструкция опухолей, лазерная коагуляция, транспупиллярная термотерапия и др. Термотерапия метод подведения тепловой энергии к тканям для локального повышения температуры, достигающей цитотоксического уровня, что в свою очередь приводит к разрушению области, которая подвергается облучению. Термотерапия может реализовываться посредством различных приборов, в том числе ультразвуковых, микроволновых, инфракрасных устройств. Лазер-индуцированная термотерапия это минимально инвазивный метод лечения опухолей различных локализаций. Транспупиллярная термотерапия это метод гипертермии посредством использования инфракрасного излучения диодного лазера. Данный метод особенно эффективен в лечении ретинобластом небольших размеров. Однако до настоящего времени нет единых алгоритмов применения данного метода у пациентов с ретинобластомой. The emphasis in the management of retinoblastoma has shifted in recent years from the frequent use of external beam radiation therapy to the widespread implementation of chemoreduction and focal therapy. This treatment strategy has been used as a way to decrease the incidence of second malignancies in children with germinal retinoblastoma. Specialists use various techniques, including systemic and periocular chemotherapy, intraarterial chemotherapy, plaque radiotherapy, laser photocoagulation, cryotherapy, and thermotherapy. Thermotherapy is a method of delivering heat to tissue to raise local temperature to cytotoxic levels, leading to tissue death. Thermotherapy can be generated with ultrasound, infrared radiation, microwave devices, and other methods. Laser-induced interstitial thermotherapy is a minimally invasive treatment for tumors in the brain, prostate, liver, and eye. Transpupillary thermotherapy is a specific form of ocular thermotherapy using infrared radiation (diode laser). It has been the most effective for small retinoblastomas. However, there are no common techniques for this method.
