Abstract

Цель исследования: оценить вероятность развития акушерских и перинатальных осложнений при индуцированных родах в зависимости от срока беременности.Материалы и методы. Проведен ретроспективный анализ акушерских и перинатальных исходов по данным 850 историй родов (форма № 096/у) за 2015–2019 гг. в ГУ «Республиканский научно-практический центр «Мать и дитя». В основную группу вошли 742 пациентки с индуцированными родами. Группу сравнения составили 108 пациенток с самостоятельно развившейся регулярной родовой деятельностью.Результаты. Проведена оценка влияния срока беременности при индуцированных родах на акушерские и перинатальные исходы. При индукции родов в сроке 41/0 неделя и более выше риски развития следующих акушерских и перинатальных осложнений по сравнению с индукцией родов в 39/0–39/6 недель беременности: первичная слабость родовой деятельности – OR 3,2 (95% ДИ 1,3–7,9), дистресс плода в родах – OR 2,7 (95% ДИ 1,3–5,8), экстренное родоразрешение путем операции кесарева сечения – OR 2,8 (95% ДИ 1,4–5,4), крупновесный новорожденный – OR 2,6 (95% ДИ 1,1–5,9), асфиксия при рождении – OR 15,0 (95% ДИ 1,5–147,5). Заключение. Следовательно, развитие акушерских и перинатальных осложнений зависит от срока беременности при индукции родов. The purpose of the study: to assess the likelihood of developing obstetric and perinatal outcome of induction of labor, depending on the duration of pregnancy.Material and methods. A retrospective analysis of obstetric and perinatal outcomes was carried out according to 850 birth histories (form No. 096/y) for 2015-2019 in the Republican Scientific and Practical Center "Mother and Child". The main group included 742 patients with induction of labor. The comparison group consisted of 108 patients with independently developed regular labor.Results. The effect of gestational age on induced births on obstetric and perinatal outcomes is assessed. With the induction of labor at 41/0 weeks or more, the risks of developing the followingobstetric and perinatal complications are higher than with the induction of labor at 39/0-39/6 weeks of pregnancy: primary weakness of labor – OR 3.2 (95% CI 1,3–7.9), fetal distress at birth – OR 2.7 (95% CI 1.3–5.8), emergency delivery by cesarean section – OR 2.8 (95% CI 1.4–5,4), a large-weightednewborn – OR 2.6 (95% CI 1.1–5.9), asphyxia at birth – OR 15.0 (95% CI 1.5–147.5).Conclusion. Therefore, the development of obstetric and perinatal complications depends on the gestational age during the induction of labor.
