Abstract

O hemangioma capilar lobular da gravidez, conhecido também por granuloma gravidarum, é um tumor benigno fibrovascular raro da segunda metade da gestação. Típico da cavidade oral, raramente se desenvolve na fossa nasal, sendo poucos os casos descritos. Os autores apresentam o caso de uma primigesta de 39anos, grávida de 36 semanas, com granuloma gravidarum gigante da fossa nasal direita, severamente hemorrágico, com antecedentes de rinossinusite crónica e epistaxis autolimitada recorrente desde o início da gravidez. Procedeu-se a excisão do polipo sob anestesia local com cauterização, com controlo da hemorragia e cicatrização completa sem recidiva. Estas lesões são benignas e na maior parte regridem após o parto, embora possam ter dimensões clinicamente importantes com efeito de massa ou hemorragia significativas, sendo nestes casos necessária a exerése cirúrgica para controlo do quadro e para diagnóstico diferencial com tumores malignos da fossa nasal, que têm uma apresentação clínica sobreponível.
