Abstract

近年,数多くの薬剤排出系が様々な菌種に見いだされ,有力な化学療法剤耐性機構として認識されつつある。特に,臨床上問題視されているMRSAや緑膿菌などの多剤耐性菌に薬剤排出蛋白が多く存在することは注目すべきである。遺伝子解析により多くの排出蛋白のアミノ酸配列が明らかにされた。薬剤排出蛋白はいずれも細胞質膜に存在するが,薬剤排出のエネルギー源として,H+の電気化学的勾配を用いるものと,ATPを用いるものと2種類ある。また,構造上関連性のない多種の薬剤を認識し排出する多剤排出蛋白の存在が排出による薬剤耐性のひとつの特徴であるが,特異性の高い排出蛋白との間で一次配列の相同性が高く,多剤排出蛋白がどのように基質を認識しているのか興味が持たれる。抗生物質を生産する放線菌に見いだされた薬剤排出蛋白は細菌の排出蛋白と類似し,その起源のひとつと考えられる。一方,緑膿菌や大腸菌などは染色体上に内在性多剤排出蛋白をもち,これらの発現量の増加も排出耐性の一因である。今日,哺乳動物などの高等生物においても,P糖蛋白質や生体アミン輸送体といった,細菌,放線菌の薬剤排出蛋白と高い相同性をもつ薬物輸送体が普遍的に存在することが知られてきている。このように,生物に普遍的に存在する薬剤排出蛋白は,生体における異物の認識排除機構の担い手であると考えられる。
