Introdução. A apresentação clínica da trombose venosa cerebral é heterogênea. Quatro síndromes maiores foram descritas em pacientes com trombose venosa cerebral: hipertensão intracraniana isolada, déficits neurológicos focais, crises epilépticas e rebaixamento do nível de consciência. Método. Descrevemos cinco pacientes consecutivos com diagnóstico de trombose venosa cerebral, ressaltando as diferentes possibilidades de manifestação clínica e prognóstico. Discussão. O diagnóstico de trombose venosa cerebral deve ser considerado em pacientes com cefaleia aguda, subaguda ou crônica, com ou sem sinais de hipertensão intracraniana ou sinais focais, mesmo na ausência de fatores de riscos para doença cerebrovascular. O tratamento deve ser iniciado imediatamente após o diagnóstico e inclui reversão quando possível dos fatores precipitantes, controle de crises epilépticas e da hipertensão intracraniana e uso de antitrombóticos.
