
La respuesta de sobresalto está compuesta por un conjunto de comportamientos reflejos destinados a preparar al organismo para enfrentar un estímulo potencialmente relevante. Esta respuesta puede ser modulada por varios factores como, por ejemplo, presentaciones repetidas del estímulo (habituación al sobresalto), o por presentación previa de un estímulo débil (Inhibición de Prepulso [PPI]). Ambos fenómenos aparecen alterados en la esquizofrenia que se cree que refleja una alteración en la neurotransmisión dopaminérgica y glutamatérgica. En este trabajo analizamos si los déficits reportados están indicando un efecto transitorio restringido a la fase aguda de la enfermedad, o si refleja un biomarcador o endofenotipo más general del trastorno. Con este fin, medimos las respuestas de sobresalto en el mismo conjunto de trece pacientes con esquizofrenia con un diseño transversal en dos períodos: 5 días después del ingreso hospitalario y 3 meses después del alta. Los resultados mostraron que tanto la habituación al sobresalto como los IBP se vieron afectados en los pacientes con esquizofrenia en la etapa aguda en comparación con un grupo de control compuesto por 13 participantes sanos, y que los IBP, pero no la habituación al sobresalto, permanecieron interrumpidos cuando se registraron 3 meses después del alta. Estos datos apuntan a la consideración de los IBP, pero no a la habituación al sobresalto, como biomarcador de la esquizofrenia.
La réponse de sursaut est composée d'un ensemble de comportements réflexes destinés à préparer l'organisme à faire face à un stimulus potentiellement pertinent. Cette réponse peut être modulée par plusieurs facteurs comme, par exemple, des présentations répétées du stimulus (accoutumance au sursaut), ou par une présentation antérieure d'un stimulus faible (Inhibition des Prépulsions [IPP]). Les deux phénomènes semblent perturbés dans la schizophrénie qui reflète une altération de la neurotransmission dopaminergique et glutamatergique. Dans cet article, nous analysons si les déficits signalés indiquent un effet transitoire limité à la phase aiguë de la maladie, ou s'il reflète un biomarqueur ou un endophénotype plus général du trouble. À cette fin, nous avons mesuré les réponses de sursaut dans le même ensemble de treize patients schizophrènes avec un plan transversal à deux périodes : 5 jours après l'admission à l'hôpital et 3 mois après la sortie. Les résultats ont montré que l'accoutumance aux sursauts et aux IPP était altérée chez les patients schizophrènes au stade aigu par rapport à un groupe témoin composé de 13 participants sains, et que l'accoutumance aux IPP mais pas aux sursauts restait perturbée lorsqu'elle était enregistrée 3 mois après la sortie. Ces données indiquent la prise en compte de l'IPP, mais pas de l'accoutumance aux sursauts, en tant que biomarqueur de la schizophrénie.
The startle response is composed by a set of reflex behaviors intended to prepare the organism to face a potentially relevant stimulus. This response can be modulated by several factors as, for example, repeated presentations of the stimulus (startle habituation), or by previous presentation of a weak stimulus (Prepulse Inhibition [PPI]). Both phenomena appear disrupted in schizophrenia that is thought to reflect an alteration in dopaminergic and glutamatergic neurotransmission. In this paper we analyze whether the reported deficits are indicating a transient effect restricted to the acute phase of the disease, or if it reflects a more general biomarker or endophenotype of the disorder. To this end, we measured startle responses in the same set of thirteen schizophrenia patients with a cross-sectional design at two periods: 5 days after hospital admission and 3 months after discharge. The results showed that both startle habituation and PPI were impaired in the schizophrenia patients at the acute stage as compared to a control group composed by 13 healthy participants, and that PPI but not startle habituation remained disrupted when registered 3 months after the discharge. These data point to the consideration of PPI, but not startle habituation, as a schizophrenia biomarker.
