Abstract

Introduccion. La identificacion de identidades reversibles o tratables en el sindrome demencial es importante. Por desgracia, las demencias no son reversibles en todos los casos, pues en muchas ocasiones el paciente no recupera el nivel intelectual previo; sin embargo, ello no significa que la enfermedad no sea tratable y que no se pueda obtener una mejoria parcial o prevenir y detener la progresion secundaria del deficit cognitivo. Desarrollo. La prevalencia de las demencias reversibles es en promedio del 15%. Los estudios rutinarios se realizan para la busqueda de entidades tratables e incluyen la realizacion de biometria hematica, velocidad de sedimentacion globular, quimica sanguinea, pruebas de funcion hepatica, determinacion serica de los niveles de vitamina B12, folatos y electrolitos sericos, examen general de orina y serologia para sifilis. Pruebas especiales como perfil hormonal (en particular, pruebas de funcion tiroidea), electroforesis de proteinas e inmunoglobulinas sericas, serologia para virus de inmunodeficiencia humana, busqueda de metales pesados, electrocardiograma, examen del liquido cefalorraquideo y rayos X de torax se deben realizar de acuerdo al contexto clinico del enfermo. Se recomienda realizar de manera rutinaria estudios de neuroimagen; la tomografia axial computarizada sin contraste seria el estudio inicial. La resonancia magnetica es mas util para la busqueda de alteraciones estructurales mas sutiles. El electroencefalograma, la tomografia por emision de positrones y la tomografia computarizada por emision de foton unico no se recomiendan como estudios de rutina. La biopsia cerebral esta indicada solo en algunos casos. Conclusion. Las causas mas frecuentes de demencias reversibles y tratables incluyen alteraciones toxicometabolicas, infecciones, uso de farmacos, la hidrocefalia obstructiva, enfermedades psiquiatricas, autoinmunes y vasculares, entre otras.
