Intracranial arteriosclerosis: determinants and clinical consequences

Abstract

Vaatverkalkingen, of arteriosclerose, zijn afzettingen van mineralen in het vaatstelsel, meestal in de vorm van hydroxyapatiet. In de vaatwant komen ze met name voor in atherosclerotische plaque in de tunica intima, of in de omliggende lamina elastica interna (IEL) of tunica media. In Hoofdstuk 1 van dit proefschrift wordt een overzicht gegeven van de beschikbare kennis over intracraniële arteriosclerose en beschrijft dat er een opvallende schaarste is aan kennis over arteriosclerose in de vertebrobasilaire arteriën (VBAC). Hierom hebben we in Hoofdstuk 2 de prevalentie van en de risicofactoren voor VBAC onderzocht, waarin we vonden dat het bij ongeveer 25% van de patiënten voorkwam. Met betrekking tot risicofactoren vonden we dat, naast leeftijd, obesitas sterk geassocieerd was met VBAC. Pseudoxanthoma elasticum (PXE) is een ziekte waarbij veel vaatverkalking voorkomt. PXE wordt in de medische literatuur vaak aangehaald als modelziekte om de effecten van geïsoleerde IEL verkalking te bestuderen. In Hoofdstuk 3 voerden we een patiënt-controle studie uit waarin we de belasting van intracraniële atherosclerotische laesies in de slagaders van de Cirkel van Willis vergeleken tussen PXE-patiënten en leeftijd- en geslacht gematchte controles. We vonden dat de belasting en verdeling van intracraniële atherosclerotische laesies vergelijkbaar waren tussen PXE-patiënten en controles. Daarnaast is het zo dat er gerandomiseerde klinische trials worden gepland in PXE patiënten om medicatie te testen die vaatverkalking afremt. Daarom voerden we in Hoofdstuk 4 een onderzoek uit met herhaalde metingen van verkalkingen door het gehele lichaam bij PXE, waarin we de specifieke ontwikkeling van vaatverkalking over de tijd hebben gekarakteriseerd. Dit is kritieke informatie voor de juiste planning en evaluatie van de eerder genoemnde gerandomiseerde studies. In Deel 3 werd een poging gedaan om intracraniële arteriosclerose verder aan te tonen als een belangrijke risicofactor voor beroerte en dementie. In Hoofdstuk 5 is specifiek gekeken naar subtypes van intracraniële verkalking van de a. carotis (ICAC), namelijk atherosclerotisch intima-verkalking versus verkalking in de IEL, en het risico op beroerte. We vonden dat beide subtypes het risico op een beroerte verhoogden, waarbij verkalking van de IEL het grootste risico met zich meedroeg. In Hoofdstuk 6 is zowel gekeken naar ICAC, ICAC subtypes, en VBAC en het risico op dementie. We vonden dat beide subtypes van ICAC en alleen grote hoeveelheden VBAC het risico op dementie verhogen. Daarnaast vonden we middels een mediatie-analyse dat een deel van dit risico werd verklaard door toegenomen schade aan de kleine vaten (witte stof hyperintensiteiten) als mediator. In de toekomst zou meer robuust bewijs met betrekking tot de vermeende causale verbanden tussen intracraniële arteriosclerose en klinische uitkomsten die blijken uit dit proefschrift moeten volgen uit goed opgezette gerandomiseerde klinische onderzoeken waarbij intracraniële arteriosclerose wordt behandeld.
