Abstract

eISSN 2093-6338 성기반응물질로 염증의 영향을 받으며 트랜스페린 포화도는 하루 변이와 영양상태 등이 검사값에 영향을 미치므로 해석 시 한계점 이 있다. 또한 만성병빈혈과 철결핍빈혈이 혼재하는 경우 만성병빈 혈과 동일하게 낮은 혈중 철 농도와 트랜스페린 포화도를 나타내 며 만성질환에서 흔히 동반되는 염증에 의해 혈중 페리틴이 상승 하기도 하므로 만성병질환과 감별하는 것이 어렵다. 최근 철결핍이 동반된 만성병빈혈과 단순 만성병빈혈을 감별하는 데 있어서 철의 체내 이용을 조절하는 hepcidin의 유용성이 제기되었는데 hepcidin은 간에서 합성되는 펩타이드 호르몬으로 십이지장에서는 철 의 흡수를 억제하고 세망내피계로부터는 철의 동원을 막는 역할 을 한다[2, 3]. 혈중 hepcidin의 농도는 간에서의 합성에 따라 결정 되는데 철 과다 시는 합성이 증가하고 철 결핍 시는 합성이 감소된 다. 또한 빈혈이나 염증도 합성에 영향을 미치는데 염증이 있는 경 우는 합성이 증가되며 빈혈이 있는 경우는 합성이 반대로 감소한 다[4-6]. 혈중 hepcidin은 전구체인 pro-hepcidin에서 분해된 형태 들로 존재하며 hepcidin-20, hepcidin-22 및 hepcidin-25가 주요 형 태들이다. 이 중 84개의 아미노산으로 구성된 hepcidin-25가 가장 강력한 활성 물질이나 높은 cysteine의 양으로 인한 안정적인 항체 분리의 어려움이 있어 최근 효소면역법을 이용한 hepcidin-25 측 서 론
