Abstract

El accidente cerebrovascular (ACV) es la primera causa de discapacidad y la segunda causa de mortalidad a nivel mundial. De entre sus etiologías, >70% de los ACV son de tipo isquémico. Debido al impacto del ACV isquémico agudo en la salud pública, se pretende que todos los médicos, incluyendo a los profesionistas en formación, conozcan el manejo del ACV claramente. Ya que el pronóstico depende en gran medida de la demora en la instauración del tratamiento, es primordial la atención médica temprana. El manejo durante la fase aguda de esta enfermedad potencialmente mortal se basa en el diagnóstico y la terapia de reperfusión aguda. Entre los estudios de neuroimagen, la tomografía computarizada (TC) es de las más empleadas por su accesibilidad y bajos costos, si bien cada vez es más amplio el uso de otras modalidades con mejor rendimiento diagnóstico. En cuanto al tratamiento de reperfusión, este tiene dos vertientes principales, la trombólisis farmacológica y la terapia endovascular (TEV). Ambas con sus correspondientes indicaciones, restricciones y ventanas terapéuticas.
