Hipercalcemia, tirando del hilo

Abstract

La sarcoidosis es una enfermedad inflamatoria que afecta principalmente a adultos de 50 a 70 años, carac­terizada por la formación de granulomas no necrotizantes causantes de fibrosis pulmonar o afecciones sis­témicas, siendo a veces sintomática o encontrada incidentalmente. Presentamos a un paciente de 51 años ingresado por hipercalcemia severa e insuficiencia renal detectadas rutinariamente, presentando síntomas como prurito nocturno, astenia e insomnio. Los análisis revelaron insuficiencia renal, hipercalcemia, eleva­ción de enzima convertidora de angiotensina, y vitamina D elevada con PTH disminuida. Con los hallazgos de pruebas complementarias se orientó como diagnóstico principal a sarcoidosis estadio III con hipercal­cemia secundaria.
