Abstract

La epidemiología global de las infecciones por Haemophilus ducreyi está poco documentada debido a las dificultades para confirmar los diagnósticos microbiológicos. Evaluamos los datos publicados sobre la proporción de úlceras cutáneas genitales y no genitales causadas por H. ducreyi antes y después de la introducción del tratamiento sindrómico para la enfermedad ulcerosa genital (GUD). Antes del año 2000, la proporción de GUD causada por H. ducreyi oscilaba entre el 0,0% y el 69,0% (35 estudios en 25 países). Después de 2000, la proporción varió de 0.0% a 15.0% (14 estudios en 13 países). Por el contrario, H. ducreyi ha sido identificado recientemente como un agente causante de úlceras cutáneas en niños de las regiones tropicales; las proporciones oscilaron entre el 9,0% y el 60,0% (6 estudios en 4 países). Concluimos que, aunque ha habido una reducción sostenida en la proporción de GUD causada por H. ducreyi, esta bacteria se reconoce cada vez más como una causa importante de úlceras cutáneas no genitales.

L'épidémiologie mondiale des infections à Haemophilus ducreyi est mal documentée en raison des difficultés à confirmer les diagnostics microbiologiques. Nous avons évalué les données publiées sur la proportion d'ulcères cutanés génitaux et non génitaux causés par H. ducreyi avant et après l'introduction de la prise en charge syndromique de l'ulcère génital (GUD). Avant 2000, la proportion de troubles gastro-intestinaux causés par H. ducreyi variait de 0,0 % à 69,0 % (35 études dans 25 pays). Après 2000, la proportion variait de 0,0 % à 15,0 % (14 études dans 13 pays). En revanche, H. ducreyi a récemment été identifié comme un agent causal des ulcères cutanés chez les enfants dans les régions tropicales ; les proportions variaient de 9,0 % à 60,0 % (6 études dans 4 pays). Nous concluons que, bien qu'il y ait eu une réduction soutenue de la proportion de GUD causée par H. ducreyi, cette bactérie est de plus en plus reconnue comme une cause majeure d'ulcères cutanés non génitaux.

The global epidemiology of Haemophilus ducreyi infections is poorly documented because of difficulties in confirming microbiological diagnoses. We evaluated published data on the proportion of genital and nongenital skin ulcers caused by H. ducreyi before and after introduction of syndromic management for genital ulcer disease (GUD). Before 2000, the proportion of GUD caused by H. ducreyi ranged from 0.0% to 69.0% (35 studies in 25 countries). After 2000, the proportion ranged from 0.0% to 15.0% (14 studies in 13 countries). In contrast, H. ducreyi has been recently identified as a causative agent of skin ulcers in children in the tropical regions; proportions ranged from 9.0% to 60.0% (6 studies in 4 countries). We conclude that, although there has been a sustained reduction in the proportion of GUD caused by H. ducreyi, this bacterium is increasingly recognized as a major cause of nongenital cutaneous ulcers.
