Abstract

Apres une longue periode d'ignorance, puis une periode au cours de laquelle les lipides ont ete consideres comme negatifs, les acides gras ont ete classes en bons ou mauvais en fonction de leur degre d'insaturation, voire en fonction de leur origine animale ou vegetale. Aujourd'hui il apparait que leurs effets sont complexes en fonction de leur dose, de leurs interactions entre eux et plus encore de leur nature biochimique precise. Si l'exces d'acides gras satures parait deletere, des apports a dose nutritionnelle ne le sont pas, et les effets des acides gras satures sont tres variables selon leur nature et donc selon les acides gras satures. Les acides gras monoinsatures continuent a beneficier de la neutralite mais leur benefice pourrait etre davantage attribue a l'environnement chimique de l'aliment source (huile d'olive) ou au mode alimentaire global associe (regime mediterraneen). La controverse est forte en ce qui concerne les acides gras polyinsatures omega 6 : certes ils abaissent, de meme que les acides gras monoinsatures, le cholesterol LDL, mais des apports trop eleves ne semblent pas associes a un benefice cardiovasculaire surtout si l'apport en acides gras omega 3 est bas. Les acides gras omega 3, tant l'acide alpha-linolenique que les AGPI a longue chaine, exercent des effets protecteurs cardiovasculaires, par des mecanismes multiples ne passant pas par les lipoproteines. Enfin une breve mise au point concerne les graisses laitieres et leur impact cardiovasculaire, loin d'etre negatif a dose moderee.
