Colestasis intrahepática del embarazo

Abstract

La colestasis intrahepática del embarazo (CIE) es la enfermedad hepática específica del embarazo más común que típicamente se presenta en el tercer trimestre. La etiopatogenia de la colestasis intrahepática del embarazo incluye factores genéticos y ambientales. Se presenta debido a que los ácidos biliares en la sangre materna y fetal aumentan, causando prurito en la madre y un mayor riesgo de resultados perinatales adversos, incluyendo parto pretérmino espontáneo, tinción meconial del líquido amniótico y muerte fetal.La confirmación diagnóstica se realiza con aumento de bilirrubinas en sangre materna y se trata con ácido ursodeoxicólico aunque no disminuya la morbilidad y mortalidad perinatal. Los estudios hoy disponibles para la valoración del bienestar fetal no son útiles para predecir el riesgo de muerte del feto, pues parece claro que ésta ocurre debido a la hipoxia fetal aguda súbita. El objetivo de este escrito es presentar una revisión actualizada de la etiología, diagnóstico y manejo de esta enfermedad.
