Priapismo

Abstract

El priapismo masculino, es una patología que se describe como una erección sostenida, dolorosa y con una duración mayor o igual a cuatro horas. Existen tres tipos de presentaciones clínicas de la patología: i) el priapismo isquémico o de bajo flujo, en este tipo de priapismo el examen físico presenta usualmente una erección rígida muy dolorosa y un glande no endurecido, ii) el priapismo no isquémico o conocido como de alto flujo el cual se asocia con historia de trauma perineal como, por ejemplo, heridas en silla de montar o trauma coital y iii) priapismo recurrente, el cual es común en pacientes drepanocíticos. La utilización de exámenes de laboratorio cuando hay duda sobre el tipo de cuadro priápico es fundamental, se deben tomar los gases sanguíneos cavernosos. Esta prueba es sumamente especifica y se utiliza para poder diferenciar el cuadro por sus valores específicos como es el Ph sanguíneo, CO2, y saturación de oxígeno. Adicionalmente se podrían utilizar estudios de imagenología como el ultrasonido Doppler, este estudio es de gran ayuda para poder diferenciar el tipo de priapismo. Es necesario conocer estos estudios y las diferencias clínicas que presentan los cuadros de priapismo ya que cada tipo de priapismo debe ser tratado de forma diferente y en algunos casos como una emergencia urológica que debe ser resuelta con prontitud.
