Percutaneous nephrolithotomy in the elderly.

Abstract

1982年5月以来172例に対し, 経皮的腎尿管結石摘出術を施行し, 60歳以上の症例は14例であった. 60歳未満の症例と比較し高齢者に対して本法を施行するうえでの問題点を検討した. 硬膜外麻酔下に患者をレントゲン透視台に腹臥位にねかせる. 側背部から腎盂まで経皮的に腎瘻を造設し, 内視鏡を挿入し結石を分割, 摘出する.対象となった患者は男性121名, 女性51名, 性比2.4:1であった. 60歳以上の症例では, 男性9名, 女性5名, 性比1.8:1とやや女性の比率が増加していた. 14症例のうち1名の尿管結石症例では本法で結石摘出が出来ず, 従来の尿管切石術を施行した. 基礎疾患として高血圧症と心房細動を有していた腎盂結石症例では術後心不全にて死亡した. 5mm以上の残石を認めた症例は3例 (23%) であった.入院期間は平均27.8日, 本法を開始してから退院までは平均21.5日で, 若年者に比して入院期間で5日, 本法開始から退院までで3.5日長かった.体外式超音波結石破砕装置が稼動を始め, 尿管鏡による尿管結石摘出術と本法とにより従来の手術は激減している. しかしどの方法を選択するにしても麻酔は不可欠であり, 麻酔に対する Risk は変わらない. 本法では腹臥位で施行するため呼吸抑制を引き起こしやすく, また内視鏡操作時の灌流液が静脈や粘膜から吸収され循環動態に対して負荷をかける可能性もある. 心肺機能及び腎機能の低下, もしくは予備力の低下している高齢者に対し本法を施行する場合, これらの Risk を考慮し基礎疾患との関連で適応を厳重に選択すべきである.
