Abstract

OBJETIVO: Conocer las características de los embarazos múltiples en un hospital de Lima. DISEÑO: Estudio descriptivo retrospectivo del embarazo múltiple. MATERIAL Y MÉTODOS: 222 partos múltiples atendidos en el Hospital General Nacional Arzobispo Loayza, entre el 1° de enero de 1991 y el 31 de diciembre de 1995. RESULTADOS: Hubo 219 embarazos dobles y 3 triples, con edades maternas entre 15 y 42 años. El 63.5% tuvo control prenatal adecuado, el diagnóstico fue realizado por ecografía en 73.4% en el embarazo y en el parto el 15.3%. Existió antecedente de gestación múltiple en 29.7%. En 49.1%, la gestación no llegó a término y las patologías asociadas con mayor frecuencia fueron anemia, preeclampsia y amenaza de labor pretérmino. El embarazo terminó por cesárea en 113 gestantes, debido a malposición fetal (38.5%), preeclampsia (25,5%) y rotura prematura de membranas (11.1%) entre otras. El 79.7% tuvo un intervalo de 15 minutos o menos entre el nacimiento de ambos fetos y en 83.5% las placentas fueron biamnióticas. En el postparto se encontró anemia (18.1%), endometritis (4.5%) infección urinaria (3.6%) y se realizó dos histerectomías puerperales por atonía uterina. Las complicaciones neonatales más frecuentes fueron: membrana bialina (10.7%) y la ictericia fisiológica (7.6%); con una tasa de mortalidad perinatal de 123 por mil nacidos vivos.
