The pengkajian process is the first stage of the maintenance process that is done systematicallyby collecting individual data comprehensively related to biological, psychological, social, andspiritual aspects. Tujuan peresanikan ini adalah untuk mengetahi keperawatan proesospengkajian keperawatan di ruang rawat inap integrated RSUP H. Adam malik medan. Penelitianini menggunakan approach studi phenomenologi. Partisispan pada peresinikan ini semanta limapartisipatan dan selected dengan methode purposive sampling. Data collection is done by usingquestionnaire data demographics, observations, and in-depth interviews and recorded usingtools. Analyze data done with the Colaizzi method. The research results were divided into fourcategories, namely the pendorong factor of pengkajian implementation, the method of datacollection in pengkajian, the benefit of pengkajian implementation, and the penghambat factorof pengkajian implementation process. Mengingat keperawatan pengkajian maka recommendedagar mendat keperahensif pengkajian keperawatan yang komperhensif dan berkesinambungan.
