El onfalocele es un defecto congénito de la pared abdominal anterior que permite la herniación de vísceras cubiertas por membrana. Se presenta el caso de un recién nacido masculino pretérmino de 32 semanas, con diagnóstico prenatal de onfalocele gigante, quien al nacimiento presentó dificultad respiratoria progresiva que requirió ventilación mecánica invasiva y surfactante, con buena respuesta y extubación temprana. El defecto abdominal fue manejado inicialmente con sistema de presión negativa, dado que no era posible un cierre primario. No se identificaron malformaciones cardíacas mayores, salvo meso-dextrocardia y leve disfunción ventricular izquierda. El manejo incluyó estabilización hemodinámica, soporte nutricional parenteral y profilaxis antibiótica. El paciente evolucionó clínicamente estable, sin datos de sepsis ni complicaciones hemodinámicas, y fue remitido a centro de mayor complejidad para resolución quirúrgica definitiva. Este caso busca destacar el manejo inicial integral en neonatos con onfalocele gigante, clave para favorecer el pronóstico frente a un defecto de alta complejidad.
