Factores asociados y respuesta clínica en el tratamiento del hombro doloroso mediante bloqueo del nervio supraescapular

Abstract

El bloqueo del nervio supraescapular (BNSE) es una técnica eficaz en el manejo del hombro doloroso. El objetivo del trabajo es evaluar el dolor y funcionalidad del hombro antes y después de un BNSE y analizar qué factores influyen en dichos cambios. Estudio de cohortes prospectivo de pacientes tratados con BNSE por dolor de hombro crónico. Se recogieron (1) factores relacionados con pacientes y dolor, (2) dolor (EVA nocturna, en reposo y durante la actividad) y funcionalidad (QuickDASH), en situación basal y al mes del BNSE, y (3) evolución tras BNSE. Análisis estadístico mediante la prueba T de Student, ANOVA y coeficiente de correlación de Pearson. Se reclutaron 54 pacientes, 29 hombres (53,7%), con edad media de 54,47 años (DE 9,82). La media de EVA antes y después del BNSE con su significación fue: nocturna 5,42 (DE 2,15) / 2,56 (DE 2,29), p = 0,003; reposo 3,36 (DE 2,42) / 0,93 (DE 1,40), p = 0,096; actividad 6,34 (DE 9,48) / 2,94 (DE 1,97), p = 0,811; y del QuickDASH 62,50 (DE 18,22) / 42,22 (DE 21,73), p < 0,001. La mejoría en EVA nocturna se relacionó con el número total de BNSE (p = 0,033) y se acercó a la significación con el tiempo de evolución del dolor (p = 0,062). El dolor nocturno y la funcionalidad del hombro mejoran tras un BNSE. La mejoría en la EVA nocturna se relaciona con el número de BNSE realizados y se ve influenciada por el tiempo de evolución de la clínica.
