Manejo clínico y cuidado del neonato con gastrosquisis

Abstract

La gastrosquisis se ubica entre las raras malformaciones congénitas de la pared abdominal, distinguida por la protrusión intestinal del abdomen fetal, en la base lateral derecha del cordón umbilical y sin un saco de cobertura. La prevalencia actual por gastrosquisis ha aumentado, al haberse determinado una frecuencia estimada de entre 2 a 3 casos por cada 10.000 nacimientos vivos, y al parecer, en los próximos años, estos valores podrían aumentar. Las razones del incremento en la prevalencia de esta patología están siendo investigadas actualmente. El objetivo del presente estudio es aportar información actualizada que refiera criterios expertos entorno al manejo clínico y cuidado del neonato con gastrosquisis. En tal sentido, se adelantó una investigación de diseño documental, aplicando una metodología de revisión, por medio de la cual se pudieron obtener los resultados que se evidencian con la exposición de definiciones fundamentales y caracterización de la gastrosquisis, datos clínicos recientes, manejo (tratamiento) clínico y cuidados de los recién nacidos (neonatos) con gastrosquisis. Se concluye que, en general, la gastrosquisis requiere un manejo multidisciplinario. El manejo clínico del neonato con gastrosquisis implica, primordialmente, que se debe atender su estabilización para así poder iniciar con la reparación del defecto de la pared abdominal, a su vez debe tener como claros objetivos: la reducción de las vísceras herniadas hacia la cavidad abdominal y el cierre de la fascia y la piel, procurando generar una pared abdominal sólida, con satisfactorios resultados estéticos, sin afectar el estado respiratorio del paciente, el retorno venoso o la vasculatura intestinal; sin descuidar paralelamente la identificación y tratamiento de las anomalías asociadas.
