Uma Causa Pouco Comum de Trombose da Veia Porta em Adolescente

Abstract

A síndrome anti fosfolipídica é uma entidade autoimune rara em idade pediátrica, sendo a trombose venosa a sua manifestação mais comum. Apresenta-se o caso de uma adolescente de 14 anos de idade com antecedentes pessoais e familiares irrelevantes, sob contracetivo oral iniciado quatro meses antes do internamento, com dor abdominal com 10 dias de evolução. No exame objetivo apresentava dor à palpação da região epigástrica e periumbilical, sem reação peritoneal. A avaliação analítica sumária não apresentava alterações. A ecografi a e posteriormente tomografia computorizada abdominal documentaram trombose venosa de todo o sistema porta, tendo sido iniciada anti coagulação. Da investi gação subsequente destaca-se a positividade para anti corpos anti fosfolípidos em duas determinações com intervalo de 12 semanas, confi rmando o diagnóstico de síndrome anti fosfolipídica. O caso apresentado destaca a trombose venosa como diagnósti co pouco comum perante um quadro de dor abdominal. A evidência de fenómenos trombóti cos implica necessariamente a inclusão da síndrome anti fosfolipídica no diagnósti co diferencial, independentemente da idade.
