Abstract

Criança de 30 meses, sexo feminino, seguida em consulta de Pediatria por malformações dos membros detetadas à nascença. Antecedentes familiares de ectro/sindactilia das mãos/pés na irmã, mãe e múltiplos familiares em 2º/3º grau da linhagem materna, sem diagnóstico estabelecido. À observação salienta-se ectrodactilia/sindactilia bilateral dos pés (Figura 1), duplicação da falange terminal do 1º dedo das mãos (Figura 2), tricodisplasia, hipotricose e hipodontia com incisivos conóides. Apresenta défice auditivo neurossensorial e desenvolvimento estaturo-ponderal e psicomotor adequado. Colocou-se a hipótese de síndrome ectrodactilia-displasia ectodérmica-fenda labiopalatina (EEC), posteriormente confirmada. Atualmente mantém seguimento multidisciplinar. A Síndrome EEC é uma doença rara1, autossómica dominantede expressão variável2. Resulta de mutações ao nível do cromossoma 3 e 73, relacionadas com o desenvolvimento da ecto-mesoderme4. A ectrodactilia/sindactilia são os achados clínicos mais comuns (85-90%), daí também se designar por síndrome da “garra de lagosta”5. As alterações ectodérmicas mais frequentes são tricodisplasia, hipotricose, distrofia ungueal e alterações dentárias. A fenda lábio-palatina surge em 60-70% dos casos4,5. Alterações oculares (alterações do aparelho lacrimal, queratites, úlceras), otológicas e do sistema nervoso central podem estar presentes, mas são menos específicas4,5. Na maioria das vezes o diagnóstico é clínico, dispensando-se o estudo genético. O acompanhamento deve ser, como no presente caso, multidisciplinar4.
