Radiografia Torácica em Crise Asmática

Abstract

Criança de oito anos, asmática que iniciou de forma súbita opressão torácica, disfagia e cansaço, motivo pelo qual recorreu ao serviço de urgência. Ao exame físico apresentava tiragem intercostal, polipneia e edema supraclavicular à direita crepitante. A auscultação pulmonar revelava murmúrio vesicular diminuído bilateralmente, aumento do tempo expiratório com sibilos dispersos. A radiografia (Rx) de tórax demonstrou sinais de pneumomediastino (figura 1), pelo que iniciou repouso e tratamento com corticoide e aerossois broncodilatadores. Por ausência de melhoria clínica às 72 h realizou tomografia computorizada (TC) torácica que documentou ar extra- luminal à volta do esófago distal e estômago proximal (figura 2). Foi observado por cirurgia pediátrica que excluiu clinicamente a hipótese de rotura esofágica distal. Continuou terapêutica dirigida a crise asmática com melhoria clínica e radiológica. O pneumomediastino espontâneo é uma entidade rara e autolimitada, afectando 0,3 % das crianças asmáticas1. Disfagia, opressão torácica, edema subcutâneo crepitante são boas pistas para a suspeita de pneumomediastino e portanto para a realização de Rx tóracico2. O pneumoperitoneo é um achado benigno e autolimitado que poderá coexistir com o pneumomediastino 3,4. A TC não costuma apresentar achados relevantes2,5, reservando-se para casos com importante compromisso respiratório e/ou hemodinâmico.
