Tumefacção cervical em adolescente: quando a citologia por biópsia aspirativa não é suficiente

Abstract

As tumefacções cervicais são uma queixa comum em Pediatria e as adenopatias reactivas constituem a etiologia mais frequente. Contudo, o diagnóstico diferencial é extenso e devem excluir-se sempre lesões graves como neoplasias malignas. Podem ser necessários exames invasivos (biópsia aspirativa ou excisional) para definir a etiologia. Apresenta-se o caso de uma adolescente de catorze anos com tumefacção cervical com quatro anos de evolução, cuja etiologia apenas foi possível conhecer pelo exame anatomopatológico do gânglio excisado. A doença de Castleman é uma doença benigna, que pode ser causa de tumefacção cervical por envolvimento ganglionar. A escassa frequência e a baixa especificidade da clínica e de alguns exames complementares (imagiologia e biópsia aspirativa) dificultam o diagnóstico. A biópsia excisional teve, neste caso, um papel primordial.
